Statement of Consent (relevant for child / young person sign ups)

By completing this form I give permission for my child(ren) to attend relevant MCF youth / childrens groups.
Activities typically remain on MCF Church property or the local area (Eg Martock Rec) unless further consent is requested in advance. 

In case of illness or accident, I authorise:

• Trained leaders to administer first aid if necessary.

• Leaders to sign on my behalf any written form of consent if required by medical authorities, once every effort has been made to contact me.
